By no means is this definitive for max dps or anything.

Another alternative would be getting Black Ice in the Frost tree, while keeping Subversion in Blood.

On a Pale Horse will speed up leveling a lot, unless you have epic flying, then the impact is much less, still good for traveling though. I'd drop runic power mastery to get that if travel speed is important to you.

Rotation is keep Scourge Striking until diseases run out, deathcoil spam before your runic power "overflowes".

I couldn't find the same for DPS, so here's one I made, I won't include the epic blacksmithing stuff, because that is probably better across the board (and insanely expensive). The set gives 238 hit rating, which is 24 shy of the cap and that can be compensated with enchants.
